What is part two of the PIP claim form and can I get help to complete it?

The PIP 2 form
The PIP2 form includes a unique barcode and during part 1 of the process some parts of the form will have already been completed for you such as your name, address and date of birth. You must normally complete and return the form within 4 weeks, although if it’s return is delayed you can request a 2 week extension.
The PIP2 claim form looks at how your disability affects different day-to-day activities relating to daily living and mobility, with some "tick-box" questions and spaces for you to explain your need for help with each activity.
Can I get help to complete it?
Third parties can help with completion of the PIP2 but you need to submit the completed form and supporting evidence from the email address you registered with the DWP, as it is this email address which is used to verify that the application is genuine.
If you require assistance with completing the PIP2 there are various organisations that may be able to assist. For example the DWP may be able to offer a referral to their home visiting service. This service is offered to vulnerable people that cannot get assistance elsewhere to claim. You should call the relevant DWP benefit team to ask if you can be referred.
